Who do I call if I have questions?

When is the 2014 open enrollment period?
When will I receive my enrollment packet?
The packet will be mailed to your mailing/home address on record. If you do not receive your packet by your effective enrollment date, contact The Plan Benefit Team at For additional assistance, please call us at (800) 690-7731 between 7 AM and 7 PM Central Time..
When do my selections become effective?
Your effective date may vary depending on the time/date you enroll. You will receive a summary page that will have your expected effective date after the enrollment process. Please note: Payment of premium is required to start/continue coverage.
Will I have options for Dental, Vision, Disability and/or Life Insurance under the new Benefits Plan?
As plan options vary by employer and geographic regions, you may not have all options available to you. Please check with your HR representative and / plan brochure for more details.
If I already have coverage, what do I need to do?
If you are eligible to waive coverage and can demonstrate proof of existing health coverage, you may select the "Waive Coverage" option if available. Please contact your HR representative for more information related to eligibility and coverage options. Note that you will be required to provide proof of further documentation to your Benefit Plan representative.
How do I enter or add dependents for my plan or plan options?
After you select an appropriate plan and options, you will be presented a screen to add any plan dependents.
How do I enroll my domestic partner?
Your plan may allow you to enroll a domestic partner. For more details on how to do this, please contact your HR representative.
How will I know if the benefits office received my elections?
